The Villa Park Public Library is an official Passport Acceptance Facility. Passport applications are accepted by appointment only.

What to bring with you:
Applications may be filled out online at travel.state.gov and then printed single-sided. The library also provides applications which must be filled out in black ink. Do not sign the application until asked to do so by an Acceptance Agent.
Applicants and Relevant parties:
The applicant must be present. Two parents and the child(ren) must be present for all minor applications under the age of 16. See Important Information below for more details.
Valid Photo Identification:
Applicants and the parents of applicants under the age of 16 must have valid photo ID, plus a photocopy of the front and back of that ID. The library can assist you in making photocopies if needed.
Original Proof of Citizenship:
Acceptable items include: certified, original, long form birth certificates; naturalization papers; or a U.S. Passport for applicants over the age of 16 plus a photocopy of the item. Hospital and/or electronic copies of birth certificates are not acceptable. The library can assist you in making photocopies if needed.
Two separate fees are required. All payments made to the U.S. Department of State must be made by check or money order. Please visit travel.state.gov or call the library for current fees. The processing fee to the library may be paid with cash, check, or credit card.
The library does not provide photo services. A passport photo must be 2 x 2-inches in size with a white background printed on photo quality paper. The U.S. Department of State has strict photo guidelines and acceptance of your photo is always at the discretion of the U.S. passport agency. We do not recommend attempting to take your own passport photo. There are several places near the library that provide this service such as Walgreens, the Villa Park branch of AAA, Walmart, and Costco.
- If one parent is unable to attend, a notarized DS-3053 Statement of Consent form must be submitted along with a copy of the front and back of the non-applying parent’s ID.
- Acceptable forms of Identification include: a valid Illinois driver’s license, valid passport, or military ID card.
- Additional documentation is needed for births abroad as well as both domestic and foreign adoptions.
